In a media release the company stated that PricewaterhouseCoopers Advisory Services Limited has been appointed as the independent auditor to investigate the cause of the oil leak at the Pointe-a-Pierre refinery on 2014 July 29.
Following the leak at Tank MP6, in the Pointe-a-Pierre Refinery, an estimated 5,000 barrels of slop oil had seeped into the Guaracara River.
They added that the majority of the slop oil was contained within the Pointe-a-Pierre Refinery.
Petrotrin later published the findings of its preliminary investigations.
These included a visual inspection report on 2010 May 21, which revealed “minor product seepage… settlement and bulging of tank on eastern side.”
This information was not entered into the Company’s computerized maintenance system.
A preliminary inspection of the collapsed bund wall following the leak also revealed that the bund wall was not constructed according to the engineering design specifications.44
